The whole concept is that flat EKG monitor line running left to right, and right at the peak it rises into a small filled heart before dropping back down to the baseline. Three thread changes: the main pulse trace, the heart fill at the peak, and a script word running near the line. At the smallest width its 1.91 inches and 6,126 stitches. Stitched the 4-inch run onto a dark red cotton shirt last valentines and it looked sharp against the fabric, which honestly suprised me given how thin the satin lines are at that scale.
These sizes are taller than they are wide, going from 3.5 inches tall at the small end to 7.5 inches tall at the largest, so keep that in mind when you hoop. Press a sturdy backer behind it regardless of the fabric you choose. The satin linework sits at density 408 so its firm enough to hold shape without feeling stiff underhand. Lay water-soluble film on fleece or any knit so the thin pulse lines dont sink into the surface texture.
One customer who makes personalised nurse gifts bought this one in March and said she stitches it onto scrub tops for the hospital staff on her ward. She normally runs the 2-inch version near the collar and the 4-inch on the chest panel. Use a sharp 75/11 needle and slow your machine to around 550 SPM on the satin sections. Dont rush the small sizes or the line edges will look rough.
Stitch it on cotton, polyester, or canvas without much hassle. Pair it with a monogram block on the opposite side of a shirt front for a personalised nurse or healthcare gift. Email me if anything with the files doesnt look right and I'll get it sorted for you.

About the artist
Reyazul Masud Riham, hand-drawing every design on this site
Every design on Re Embroidery is hand-digitized by one person. Each file gets sketched, color-matched, and stitch-tested on real fabric before it earns a place in the shop. No team. No auto-conversion from images. Just slow, deliberate work, sometimes three or four days per design.
That's the joy I work for.
The hard part is finding my designs re-uploaded and resold elsewhere. So when you buy from Re Embroidery, you're paying one real person for the file you're about to download. That matters.
